=== Hormonal === Vitamin D metabolism is regulated not only by the negative feedback mechanism of calcitriol but also by two hormones: parathyroid hormone (PTH) and fibroblast growth factor-23 (FGF-23). These hormones are essential for maintaining the body's calcium and phosphate balance. Parathyroid hormone (PTH) regulates serum calcium through its effects on bone, kidneys, and the small intestine.Bone remodeling, a constant process throughout life, involves bone mineral content being released by osteoclasts (bone resorption) and deposited by osteoblasts. PTH enhances the release of calcium from the large reservoir contained in the bones. It accomplishes this by binding to osteoblasts, in this way inhibiting the cells responsible for adding mineral content to bones, thus favoring the actions of osteoclasts. In the kidneys, around 250 mmol of calcium ions are filtered into the glomerular filtrate per day, with the great majority reabsorbed and the remainder excreted in the urine. PTH inhibits reabsorption of phosphate (HPO42−) by the kidneys, resulting in a decrease in plasma phosphate concentration. Given that phosphate ions form water-insoluble salts with calcium, a decrease in the phosphate concentration in plasma (for a given total calcium concentration) increases the amount of ionized (free) calcium. A third important effect of PTH on the kidneys is stimulation of the conversion of 25-hydroxy vitamin D into 1,25-dihydroxy vitamin D (calcitriol). After six years in Hawaii, Stalcup moved to the University of Cincinnati, where she was made full professor by 2001. Whilst in the United States, Stalcup served on the advisory board of the Greater Cincinnati Water Works. In 2012, Stalcup moved to Ireland, where she was made Director of the Irish Separation Science Cluster and Professor of Chemical Sciences at Dublin City University. Stalcup served as a council member of the Royal Society of Chemistry Analytical Division.
=== Pharmacokinetics === LGD-4033 showed linear or dose-proportional pharmacokinetics across doses of 0.1 to 1 mg/day over 21 days of administration. Levels of LGD-4033 were 3-fold higher at day 21 compared to day 1, indicating significant accumulation with repeated administration. The mean area-under-curve levels of LGD-4033 on day 21 were 19 ng•day/mL at 0.1 mg/day, 85 ng•day/mL at 0.3 mg/day, and 238 ng•day/mL at 1 mg/day. The elimination half-life of LGD-4033 is 24 to 36 hours. Pharmacokinetic studies of LGD-4033 for purposes of doping detection have also been conducted.
=== Occupational harm reduction strategies === Drug and alcohol impairment while at work increases the risk of work-place accidents and decreases productivity. Employers such as the commercial driving and airline industry may conduct random drug tests on employees with the goal of deterring use to improve safety. There is some evidence that increasing the use of random drug testing in the airline industry reduces the percentage of people who test positive, however, it is unclear if this decrease is associated with a corresponding decrease in fatal or non-fatal injuries, other accidents, number of days absent from work. It is also not clear if there are other unwanted side effects that may result from random drug and alcohol testing in the workplace.
== Medical uses == Dofetilide is used for the maintenance of sinus rhythm in individuals prone to the occurrence of atrial fibrillation and flutter arrhythmias, and for chemical cardioversion to sinus rhythm from atrial fibrillation and flutter. Based on the results of the Danish Investigations of Arrhythmias and Mortality on Dofetilide ("DIAMOND") study, dofetilide does not affect mortality in the treatment of patients post-myocardial infarction with left ventricular dysfunction, however it was shown to decrease all-cause readmissions as well as CHF-related readmissions. Because of the results of the DIAMOND study, some physicians use dofetilide in the suppression of atrial fibrillation in individuals with LV dysfunction, however use appears limited: After initially receiving marketing approval in Europe in 1999, Pfizer voluntarily withdrew this approval in 2004 for commercial reasons and it is not registered in other first world countries. It has clinical advantages over other class III antiarrhythmics in chemical cardioversion of atrial fibrillation, and maintenance of sinus rhythm, and does not have the pulmonary or hepatotoxicity of amiodarone, however atrial fibrillation is not generally considered life-threatening, and dofetilide causes an increased rate of potentially life-threatening arrhythmias in comparison to other therapies.
=== Production === Generators provide radiation shielding for transport and to minimize the extraction work done at the medical facility. A typical dose rate at 1 metre from 99mTc generator is 20–50 μSv/h during transport. These generators' output declines with time and must be replaced weekly, since the half-life of 99Mo is still only 66 hours. Since the half-life of the parent nuclide (99Mo) is much longer than that of the daughter nuclide (99mTc), 50% of equilibrium activity is reached within one daughter half-life, 75% within two daughter half-lives. Hence, removing the daughter nuclide (elution process) from the generator ("milking" the cow) is reasonably done as often as every 6 hours in a 99Mo/99mTc generator.
